Admission, Scholarships and Accommodation Application – Declaration and authority

Your admission, scholarships and accommodation application ('application') cannot be considered unless you or, if applicable, the person you have authorised to act on your behalf accept all conditions set out in the declarations below. An authorised person accepting all conditions set out in the declarations below — on your behalf — is tantamount to you accepting these conditions.

I certify that I have read and understood the questions on the application and conditions on this declaration and that my answers are true and correct. The Australian National University ('ANU') collects information about you, including information you provide in this application to assist in the assessment of your application.

1. General Declarations & Applicant Responsibilities

1.1 I declare that all the information I have submitted through this application is true and complete and not false and/or misleading. I understand giving false and/or misleading statements, or submitting fraudulent documentation in support of an application (see clause 4) for the purpose of gaining admission to the University is an offence under Australian law. I understand that this may result in my application being rejected or, if any offer had been made, it will be withdrawn or, if I have been enrolled, my enrolment will be cancelled.

1.2 I understand that ANU will engage and utilise the assistance of The Universities Admissions Centre ('UAC') in processing this application. This includes the collection of personal information (see clause 5), assessment and auditing of my application. For such purposes, I acknowledge that any reference to ANU includes UAC.

1.3 I declare that I have read and that I understand the statement on fraudulent, untrue or incomplete information submitted by applicants' (see clause 4 below).

1.4 I declare that I have read and that I understand the statement on 'collection of personal information' (see clause 5 below).

1.5 I understand that ANU may use my personal information to progress my application. Furthermore, I acknowledge that ANU may, from time to time, send me information about events and activities related to studying there. (Note: you can opt out of receiving such emails at any time).

1.6 I understand that ANU follows certain standards for coursework student admissions. I declare that I have read the relevant ANU policy and procedure.

1.7 I understand that scholarships are awarded pursuant to specific conditions of award that are attached to each scholarship and the relevant ANU policy and procedure.

1.8 I authorise ANU to obtain available official records from any educational institution attended by me. I understand that ANU is not responsible if any institution does not provide these records.

1.9 I understand that, if applicable, ANU may check my visa and residency status using Visa Entitlement Verification Online (VEVO). As such, I authorise ANU to do so.

1.10 I authorise ANU and UAC, to collect, receive, store, transfer and use any information provided by me, or any information obtained in connection with this application, and to disclose such information to Universities Australia, Universities Australia member institutions, the members of the Australasian Conference of Tertiary Admissions Centres (ACTAC), the Tertiary Education Quality and Standards Authority (TEQSA), the Australian Skills Quality Authority (ASQA), State and Territory governments and any other authority or tertiary educational institution either in Australia or overseas, where ANU reasonably considers it is necessary to make such disclosure.

1.11 I authorise UAC to provide all information and documents obtained in connection with this application to ANU for any purpose connected with my possible selection, admission and/or enrolment in any course.

1.12 I understand that any information (including personal information - (see clause 5)) in or connected with this application may be shared with third parties (including donors and/or other external persons) for the purposes of scholarship assessment. I authorise ANU to do so for such purposes.

1.13 I authorise ANU to collect, receive, store, transfer and use all such information; and all information in or connected with this application for training and/or research purposes. I authorise ANU to provide personal information (including academic results) to UAC or a person nominated by UAC. I give these authorities on the condition that all information provided pursuant to training and/or research purposes is de-identified where possible.

1.14 In the event that I am enrolled at ANU, I authorise ANU to release any personal information (see clause 5) that it may hold about me to any other educational institution that is seeking to verify my student conduct or academic record for the purpose of determining my eligibility for admission or enrolment at that institution.

1.15 I understand that any information I provide to ANU, or is maintained by ANU, may be given to Australian government departments or agencies as required by law.

1.16 I understand that ANU may disclose the personal information (see clause 5) I have given in this application to the Department of Education, Skills and Employment, or its successor, for the purpose of:

use in connection with the Tertiary Collection of Student Information (TCSI) or provider registration; and/or
use in connection with the national data collection on University applications and offers; and/or
other collections as the Department of Education, Skills and Employment, or its successor, may lawfully require from time-to-time.

I also understand that the Department of Education, Skills and Employment, or its successor, may also disclose this information to the Australian Taxation Office (ATO).

1.17 I understand that it is an offence to submit fraudulent or misleading documentation in support of an application (see clause 4) for the purpose of gaining admission to ANU. I understand that 'application' in this context includes applications for admission as well as scholarships. I understand that ANU has the right to vary or cancel an application or enrolment made on the basis of what ANU determines to be untrue or incomplete or fraudulent or misleading information from any source. I understand that providing such information may lead to my application being withdrawn or denied and/or cancelled; and if enrolled, the enrolment (including any scholarship(s)) may be terminated.

1.18 I understand that in some cases ANU makes admission and scholarship offers based on the information provided by relevant third parties, such as referees, and on advice received from other relevant institutions, including UAC. If ANU subsequently finds that such information or advice was incorrect or untrue or misleading or fraudulent (see clause 4), ANU has the right to vary or cancel an application or enrolment (including any scholarship(s)) on this basis. This may result in my application being withdrawn or denied and/or cancelled; and if enrolled, the enrolment (including any scholarship(s)) may be terminated.

1.19 I understand that ANU and UAC may release details of my program preferences and offers to my school/college for assessment purposes. I authorise ANU to do so for such purposes. (Note: If you would prefer for these details not to be released please contact the ANU admissions office).

1.20 I understand that enrolment at the Australian National University is conditional upon accepting, and complying with all of the rules, policies, terms and conditions of the Australian National University, including, but not limited to, those pertaining to academic integrity.

1.21 I authorise ANU to verify any information I provide through my application. I understand that this includes auditing my application by obtaining official records from any institution I have attended or by conducting other enquiries otherwise required to verify information or documentation presented with my application, including but not limited to referee reports and undeclared study. Accordingly, I consent to ANU providing my personal information (see clause 5) to any relevant third party for these purposes. If requested, I agree to provide original documents to support my application.

1.22 I authorise:

UAC & ANU to use Centrelink Confirmation eServices to perform a Centrelink enquiry of my Centrelink Customer details and concession card status in order to enable the business to determine if I qualify for a concession, rebate or service
the Australian Government Department of Human Services (the department) to provide the results of that enquiry to UAC & ANU.

I understand:

The department will disclose personal information to UAC & ANU, including my name, address, concession card status, payment type, payment status, one-off payment, income, assets, deductions, shared care arrangements, partner status and Youth Allowance Independence rate to confirm my eligibility for the National Access Scheme.
This consent, once signed, remains valid while I am an applicant of UAC & ANU unless I withdraw it by contacting UAC & ANU or the department.
I can get proof of my circumstances/details from the department and provide it to UAC & ANU so that my eligibility for the National Access Scheme can be determined.
If I withdraw my consent or do not alternatively provide proof of my circumstances/details, I may not be eligible for the National Access Scheme.

2. Additional Tuckwell Declarations (applicable only to Tuckwell Scholarship applicants)

2.1 I understand that if my circumstances change after applying for the Tuckwell scholarship and if I subsequently decide to not proceed with the application, I will notify ANU of my withdrawal from the process.

2.2 I understand that the Tuckwell scholarship application is a separate application, and is in addition to this application. I authorise ANU to transfer any information in or connected with this application to any third party that collects, receives and stores information for the purposes of assessing my Tuckwell scholarship application.

3. Additional Accommodation Declarations

3.1 I declare that I have read, and that I understand the ANU accommodation guarantee.

3.2 I acknowledge that ANU will take effort to accommodate as many students as possible, however I understand accommodation cannot be guaranteed for some ACT applicants.

3.3 I acknowledge that if I do not disclose any medical/special requirements at this point of the application, any personal condition of mine cannot be considered after my application has been submitted.

3.4 I accept that whilst my preferences will be taken into account, ANU may offer me accommodation in another approved residence, where costs may vary, for which I am eligible. I understand that if I decline an offer of accommodation made by ANU, I will not receive another offer.

3.5 I am aware that approximate costs applicable to the student residence in which I am offered a place, will be outlined in my offer letter.

3.6 I am aware that although I have selected either catered or self-catered accommodation, I may not receive that preference (i.e. selected catered accommodation but may be offered self-catered accommodation). I understand that I need to be prepared for all types of accommodation living styles offered by ANU.

3.7 I agree to indemnify and keep indemnified ANU, ANU accommodation providers and their employees and agents against any claims or losses or injury arising out of the non-disclosure of any medical/special requirements associated with my application.

3.8 I declare the information I have supplied is correct to the best of my knowledge. I have not failed to supply any information nor provided false information. I authorise any ANU owned or affiliated residential College to obtain from my secondary/tertiary institution; personal information, contact details and results of previous study from my application for admission purposes. I understand that in this context, this information will only be used in relation to my application for accommodation.

3.9 For the purposes of my application for accommodation, I acknowledge that I have read, understood and I consent to the Privacy Act 1988 Collection Notice set out below.

Privacy Act 1988 – Collection Notice

The personal information provided by you is necessary for ANU to verify your identity and eligibility to apply for ANU residential accommodation. Personal information collected about you in this application may be compared with information already held in ANU student systems and discussed with relevant persons within ANU or external to ANU.

If you are successful in obtaining accommodation with an ANU accommodation provider, ANU will provide information about you to those accommodation providers, including your enrolment status, your academic progress and results and your contact and address details. ANU will also receive information about you from those accommodation providers, including information about your current residency status, any outstanding debts and discipline matters. The information collected and provided about you will be used solely for purposes connected to providing you with residential accommodation.

4. Statement on Fraudulent, Untrue or Incomplete Information/Documentation submitted by Applicants

ANU recognises that all decisions which affect an applicant must be made fairly using appropriate academic or other criteria as specified by relevant ANU policies and procedures. Selection into programs by application to ANU is based on merit. It is the responsibility of an applicant to ensure that they comply with all instructions when submitting an application to ANU. An application must be complete and correct and all attempts at academic study must be disclosed.

All matters concerning fraudulent, misleading or untrue documentation may be reported to the relevant Government and statutory authorities. ANU will also investigate, and may take action against, an applicant who, by any other means, has or could have achieved an advantage over other applicants.

Examples of conduct by an applicant that will be investigated by ANU include, but are not limited to:

submitting an incomplete or falsified academic transcript;
failing to declare previous study at secondary, post-secondary or tertiary level;
submitting an incorrect or otherwise misleading statement of employment experience;
claiming special consideration of educational disadvantage based on incorrect information;
having some other person complete a selection task on the applicant's behalf such as an interview, audition or a portfolio of work;
claiming membership of a special selection group when not a member of that group.

The procedures for processing applications suspected of being in some way incomplete or untrue have been approved by ANU and the NSW Vice-Chancellors' Committee. These procedures accept the right of applicants to be treated fairly and without prejudice, and in accordance with the principles of natural justice. 5. Statement on Collection of Personal Information

ANU collects personal information and, in some circumstances, information regarding your health, or information about someone other than yourself, for the purpose of processing your application for admission. If you are providing personal information about someone other than yourself, you should inform them about the ANU privacy policy.

While your personal information is generally collected from you, UAC also collects information about you from other educational institutions that you have attended. UAC provides your personal information and, where applicable, your health information, to ANU for the purposes of processing your application for admission.
