DocumentationDo I need to submit any documents with my application?In most cases UAC can obtain your tertiary results from Australian Universities, TAFE NSW and TAFE ACT (CIT) on your behalf. If UAC cannot access your results you will be advised in your Apply-By-Web application. Apply-By-Web instructions You need to print your document cover sheet at the end of your application along with your Confirmation of application. Your document cover sheet has a barcode on it and correctly verified documents (read "Who can verify copies for you?" below) must be attached to this cover sheet and forwarded to UAC by the date specified. You will have seven days to supply your documents to UAC after you have completed your application.
Tertiary studies in AustraliaIn most cases UAC can obtain your results from Australian Universities. TAFE NSW studiesIn most cases UAC can obtain your results. If your last year of attendance was 1975 or earlier you must supply correctly verified documentary evidence of your results for any course. If you completed enrolled nursing, secretarial, or trade studies before 1994 you must provide correctly verified documentary evidence. TAFE ACT (CIT)In most cases UAC can obtain your results. Other Australian TAFE studiesIf you studied at TAFE Victoria, South Australia, Northern Territory, Queensland, Tasmania or Western Australia you must supply correctly verified documentary evidence of your results, including proof of completion where appropriate. Australian Qualifications Framework courses delivered by private providersYou must supply correctly verified documentary evidence of your results, including proof of accreditation and course completion where appropriate. Documentation must indicate Australian Qualification Framework (AQF) accreditation. Your qualification will not be assessed without proof of accreditation. State-accredited courses delivered by private providersYou must supply correctly verified documentary evidence of your results, including proof of accreditation and course completion where appropriate. Documentation must indicate accreditation of the course by the relevant authority of the Australian state or territory, such as the NSW Vocational Education and Training Accreditation Board (VETAB). Your qualification will not be assessed without proof of accreditation. Secondary or Tertiary studies undertaken overseasYou must supply correctly verified documentary evidence of your results, including proof of course completion where appropriate. Professional and Paraprofessional QualificationsIf you have ever completed or expect to complete this year, or are currently undertaking, a professional or paraprofessional qualifications you must provide correctly verified documents. You must provide documentary evidence of your current Australian Nurse’s Registration. Courses not to includeThe following courses are not assessed:
For courses delivered by Australian private providers, only those courses accredited under the Australian Qualifications Framework (AQF), or by the relevant state/territory accreditation authority, will be taken into account. Qualifications undertaken with private providers will not be assessed without proof of accreditation for the particular course. Studies in 2008You may not be able to provide all documentation with your application because you may be sitting for an examination or test at the end of 2008. If you are, you must forward a correctly verified copy of your transcript to UAC as soon as you receive it. Academic records - general
A collection of examination result slips is not acceptable. Apply for your transcript/s early because there may be a delay at some institutions in obtaining official transcripts. If you have completed the course, the transcript you provide must state the award you gained. If the transcript does not state that you have successfully completed the course, you must provide additional information, for example a verified copy of your certificate or diploma.

How do I supply my documents to UAC?If "Yes" appears in the "Documents to be provided" box during your Apply-By-Web application, you will be required to print your Document Cover Sheet at the end of your application along with your Confirmation of application. Name change documentation and Statement of Services for your employment experience do not have to be verified. ^topDocument feeAfter you have completed your application, all documents that you must provide to UAC will be listed on your Document Cover Sheet. Correctly verified documents should be scheduled to arrive at UAC seven days after you have applied. $38 document fee Correctly verified documents onlySend correctly verified documents only. Read 'Who can verify copies for you?' below. However, you must be prepared to provide original documents if we ask for them. UAC does not return documents. If you send original documents and want them returned, you must write to UAC enclosing a cheque or money order for the relevant fee. If the documents relate to the current admissions period, the fee is $35. If the documents relate to a previous admissions period (up to two years) the fee is $50. ^topWho can verify copies for you?To have your original documents verified by UAC or by any participating institution you can take them in person to UAC or the Admissions Office of an institution. Your original documents will be copied and handed back to you. UAC will retain the copies with your application. The Admissions Office will give the copies to you to submit with your application to UAC. There may be a charge for copies made at some participating institutions. Alternatively, you can have your documents verified by someone who belongs to one of the categories listed below. The categories depend on whether you are having your documents verified in Australia, New Zealand or overseas - check carefully which categories apply to you. The person who verifies your document/s must be contactable by telephone during normal working hours. If you have studied under a previous nameIf you have studied under a previous name, you must supply documentary evidence satisfactory to us to prove your change of name, such as a marriage certificate, deed poll registration or other registration with the Registry of Births, Deaths and Marriages. The documentation must show your previous name/s and your current name/s. Without this documentation, your application may be delayed and extra fees may apply. ^topEmployment experienceSome institutions will take employment experience into account for admissions purposes. You must supply the correct documentation with your application in the form of a statement of service on the employer’s official stationery stating:
Personal references that do not state the above mentioned items cannot be assessed. Group certificates, offers of employment, payslips and resumes will not be accepted.
